Appointments
The clinics are open for appointments Monday through Friday from 8:30 a.m. to 5:00 p.m. We also offer convenient Saturday appointments on a monthly basis from 9:00 a.m. to 12:00 p.m. We can often accommodate same day appointments with the use of our practitioners for your urgent needs.
To provide the most efficient and best care, we see our patients by appointment. The office hours are flexible and we strive to be available at times that are accessible to you. Simply call our office and briefly explain the nature of your visit; our appointment coordinators will work to arrange an appointment that is convenient. Although we always endeavor to make your visit to our office a pleasant experience, emergencies and deliveries do arise. Our receptionist will attempt to notify you of a long or unusual delay.
To make an appointment, you will need to have your insurance card available. In order for us to put your information in our system and to verify your benefits and eligibility, we need the information listed on the right under "Prior To Appointment".
Emergency Care
A physician is on call 24 hours a day for urgent medical care. During regular office hours, emergency needs will receive immediate attention. After office hours, please call the office at 713.365.2900 for further instructions.
Prior To Appointment
Prior to your appointment, you may go to the Patient Forms and download the forms. For your convenience and time, we recommend you complete these forms prior to your appointment and bring them with you.
Patient: Full name, date of birth, social security number, address and phone number.
Insured: Full name, date of birth, social security number, relation to patient
Insurance: Group or Policy number, ID number, Member Services phone number to call to verify benefits, Name of Insurance Carrier, note whether HMO or PPO or other. If it is an HMO, we need the primary care physician's name.
Reason for visit: Annual Exam, Pregnancy, Gynecological Problem
