Patient Responsibility

Confirm, by calling the number on the back of your insurance ID card, that the physician you are seeing is "In-Network" as a contracted Provider under your plan. If you are part of an HMO, verify with your insurance that the PCP you have on file does not inhibit your ability to "direct access" our physicians without a referral. Do not rely on Provider directories, or our office staff, for confirmation of plans participation.

If we are not Providers under your plan, you are required to pay in full at the time of service. We are pleased to file the claim with your primary insurance, and will reimburse you if any benefits are paid.

You MUST bring your current insurance ID card with you. Upon each visit you will be asked to present proof of coverage. Failure to provide proof of coverage and the proper information to file with your current primary insurance may require payment in full at the time of service and each visit thereafter, until the necessary information to file your claim is received.

According to your plan, you may need a referral from your PCP, authorization from your insurance company, or other requirements to visit our physicians. This is your responsibility to investigate and obtain, and failure to do so may require payment in full at the time of service.

Well Woman, or Annual Exams, consist of a pelvic and breast exam, along with a PAP Smear. This is considered preventive care, and is not covered by many insurance policies (including Medicare). Please check with your insurance carrier prior to your visit as to whether this is a benefit under your policy. Be prepared to submit payment for the exam at the time of service iif it is a non-covered service.
